Resource Navigation
Navigating benefits, healthcare systems, and service networks can be overwhelming, especially for those already managing the effects of trauma, transition, or instability. Path of Honor Foundation offers personalized resource navigation to help veterans access the services they need with confidence and clarity.
Our team of trained staff and volunteers works one-on-one with veterans to identify needs and connect them with the right support. We assist with understanding and applying for VA benefits, scheduling healthcare appointments, accessing housing programs, finding mental health care, securing legal aid, and more.
We know that many veterans give up on seeking help due to confusing systems, long waitlists, or past negative experiences. Our goal is to simplify the process and walk alongside each veteran until they receive what they need. We don’t just hand out phone numbers or links — we make calls, attend appointments when needed, and follow up regularly to ensure follow-through.
We also build partnerships with local organizations and service providers to create a network of trusted, veteran-friendly resources. When appropriate, we help coordinate services across multiple systems to make sure nothing falls through the cracks.
Our resource navigation service is open to all veterans, regardless of discharge status or income level. We tailor our support to individual goals, whether someone is trying to avoid eviction, find a job, access medical care, or simply get organized. Veterans can schedule an intake appointment to begin the process at any time.
